Documents Required When Registering With A GP

If it is your first time of registering with a GP in this country, you will need:

  • Your passport - WITH A VALID VISA OVER SIX MONTHS
  • Proof of your current address (e.g. tenant agreement or utility bill)
  • Your birth certificate
  • Your date of arrival in the UK
  • Telephone or contact number

If you are already registered with the NHS but are changing from another practice, you will need to bring:

  • Proof of your current address (e.g. tenant agreement or utility bill)
  • Your birth certificate
  • Your medical card or, alternatively, the name and address of your previous GP
  • Details of previous address
  • Telephone or contact number
  • Your passport

Change of Address/Telephone Number

Please notify us of any change of address or telephone numbers so that we can bring your records up to date. You will need proof of your new address. Please bring a copy of a utility bill in your name.
